What to Bring —
Please bring these items with you to the consultation. You can submit the patient registration forms ahead of time and submit them online. If you do not submit the forms in advance, we ask that you arrive a few minutes early on the day of your appointment to complete these forms. If you have X-rays from another office, you can request that they be forwarded to our office before your appointment.
- Referral slip
- X-rays
- List of current medications
- Dental or medical insurance information
- Valid ID, such as a driver’s license

Parents of Minors —
Patients under the age of 18 must be accompanied by a parent or legal guardian to their consultation appointment. This ensures we have the proper consent for treatment and can address any questions or concerns directly with a responsible adult.
What to Expect —
At this visit, we will discuss your oral health concerns and goals for treatment. We may take 3D scans of the head, neck, or face, in addition to performing an oral examination. Your oral surgeon will discuss the diagnosis and options for treatment, then create a custom treatment plan unique to your needs. If surgery is necessary, we will also talk about your options for anesthesia. It’s a good idea to come prepared with a list of questions so that we can address all of them at this time.

Financial & Insurance Information —
Kitsap Oral, Maxillofacial & Dental Implant Surgery accepts many methods of payment to make care more accessible to our patients. We also accept most major insurance plans. To find out the details of your dental coverage, contact your insurance provider directly. Please remember that you are responsible for fees due at the time of service regardless of your insurance coverage unless prior arrangements were made.
